If you’re preparing to return to the community after incarceration—or navigating the pre-trial process—you don’t have to do it alone.
Our case managers are here to support you every step of the way. We begin working with you 2–3 months before your release or during your pre-trial phase to build a strong foundation for your transition.
You’ll have access to weekly support groups where you can connect with others, share experiences, and build a sense of community. We also offer positive, structured activities like volunteer opportunities, group outings, and connections to education and networking resources—helping you stay engaged and moving forward.
Together, we’ll create a personalized plan tailored to your goals and needs. Using a proven S.M.A.R.T. goal framework (Specific, Measurable, Attainable, Realistic, and Time-based), we’ll help you identify clear steps toward success and connect you with the resources you need—whether that’s housing, employment, education, or healthy relationships.
